Support at Home, self-managed with Trilogy Care

Anjelic Hearts has partnered with Trilogy Care, an approved Support at Home provider, to ensure that you have access to high-quality services via your Support at Home funding. Trilogy Care can administer your care package while you have the flexibility to work with the service providers of your choice. Both organisations are focused on empowering you to maximise your care hours and wellbeing by giving you choice, control and independence over your care.

What is Support at Home?

Support at Home is a funding program provided by the Australian Government that supports older Australians to live safely and independently in their own home. It replaced the Home Care Packages Program on 1 November 2025.

Who it's for

Older Australians who need help to keep living at home. An aged care assessment arranged through My Aged Care decides whether you’re eligible and what level of support you can receive.

What it covers

A defined list of services, including personal care, nursing, help around the house, meals, transport, social support and respite. It can also fund equipment and home modifications. Anjelic Hearts provides many of these services. See our services.

How it's funded

The Australian Government provides a budget based on your assessed needs. Depending on your income and assets, you may contribute to the cost of some services, such as help around the house. You do not contribute to clinical supports such as nursing care, and from 1 October 2026 you don’t pay a contribution for approved personal care services.

Self-management

Self-management or fully managed: what’s the difference?

Self-management is a way for you to manage the service providers you use and how your Support at Home budget is spent. It suits people (and families) who want more say in their care. If you’d rather your provider arranges everything, a fully managed approach may suit you better.

Self-managed with Trilogy Care

  • Your care plan: You design and plan your own care plan with a dedicated care partner
  • Choosing providers: You choose the service providers you would like to work with, such as Anjelic Hearts
  • Your budget: You gain more control over how your funds are spent
  • Care hours: The care hours your budget buys depend on the prices agreed with each service provider
  • Care management: Your provider still provides care management and oversight
  • Involvement: More involvement from you or your family

Fully managed

  • Your care plan: Your provider leads the planning with you
  • Choosing providers: Your provider arranges services, usually from its own staff or partners
  • Your budget: Your provider manages spending on your behalf
  • Care hours: The care hours your budget buys depend on your provider's prices
  • Care management: Your provider provides care management and oversight
  • Involvement: Less day-to-day involvement needed

Every provider sets its own prices. Ask any provider to explain its fees before you sign an agreement.

Trilogy Care administers your package

Trilogy Care, an approved Support at Home provider, administers your Support at Home package. You’ll have a dedicated care partner to help you plan your care and budget.

You choose Anjelic Hearts

With self-management, you choose the service providers you’d like to work with. When you choose Anjelic Hearts, we meet you, listen to your goals and help build a person-centred care plan.

We deliver your care

Our team provides your agreed services at home and keeps in touch with you, your family and your Trilogy Care partner as your needs change.

To apply for Support at Home, start by talking to your GP, who can refer you to My Aged Care for an assessment. Or contact My Aged Care directly to create a profile and arrange an assessment.

What is self-management?

Self-management is a way for you to manage the service providers you use and how your Support at Home budget is spent. It gives you the flexibility to:

No. You’ll have a dedicated care partner to help you plan and review your care. Under Support at Home, your provider must still provide care management, however much of your care you choose to self-manage.

Yes. You need an aged care assessment through My Aged Care. Ask your GP for a referral, or contact My Aged Care on 1800 200 422 to create a profile and arrange an assessment.

It depends on your assessed budget, the services you choose and your income and assets. You won’t pay a contribution for clinical supports such as nursing, and from 1 October 2026 you don’t pay a contribution for approved personal care services.
