About our care

We offer a range of services such as personal care, dementia care, assistance with daily living, household tasks, companionship, meal preparation, social outings, transport, medication management, and a wide range of nursing services (including wound care, continence and catheter care, in-home palliative care, health monitoring and more).

Through our partnership with Trilogy Care, you can self-manage your Support at Home package and choose Anjelic Hearts for the care and services that help you stay at home longer. We work with you to create a person-centred care plan by discussing your goals, and help you get the most out of your funding. Our services are tailored to meet your specific needs, and you can contact us about your care plan at any time.

At Anjelic Hearts, we are committed to a person-centred approach – our team dedicates time and effort to understanding your needs and goals so we can build a plan that best suits you. Creating and maintaining a strong rapport with each individual and their family is a priority, and we embrace the unique needs and cultural diversity of everyone.

Our staff undergo extensive training to ensure that we can assist each client specifically and deliver individualised care. Our team has over two decades of experience working in the aged care sector, which reflects not only our commitment, but a genuine dedication to providing safe, person-centred care that empowers older Australians to live with dignity, choice and independence.

We’ll do our best. Our team is multicultural, and we embrace the unique needs and cultural diversity of everyone we support. Tell us about your preferred language, cultural or faith practices, food and traditions, and we’ll talk with you about how we can include them in your care plan.

Our in-home supports, such as companionship, personal care and help around the house, can give family carers time to rest and look after their own needs. Talk to us about what would help your family. Under Support at Home, respite is one of the services available, depending on the assessed needs of the person receiving care.

Yes. Coming home from hospital can be a time when extra help makes a real difference. We can talk with you about support such as personal care, meals, transport to follow-up appointments and nursing care such as wound care. It helps to contact us before discharge if you can. If you’re eligible, the Support at Home Restorative Care Pathway may also provide short-term support to help you regain strength and independence. Ask your hospital team or My Aged Care.

Support at Home & funding

Yes. Anjelic Hearts has partnered with Trilogy Care, an approved Support at Home provider, to ensure that you have access to high-quality services via your Support at Home funding. Trilogy Care can administer your care package while you have the flexibility to work with the service providers of your choice.

Both organisations are focused on empowering you to maximise your care hours and wellbeing by giving you choice, control and independence over your care.

Support at Home is a funding program provided by the Australian Government that supports older Australians to live safely and independently in their own home. The program is designed to support you with daily activities you may need assistance with, such as cleaning, showering or attending appointments. Self-management is a way for you to manage the service providers you use and how your Support at Home budget is spent. It gives you the flexibility to:

  • design and plan your own care plan with a dedicated care partner
  • choose the service providers you would like to work with
  • gain more control over how your funds are spent
  • decide how your care hours are used.

Yes. As well as supporting people with Support at Home funding, we welcome private clients who pay for their own care. This can suit people who are waiting for an assessment or funding, aren’t eligible, or would like extra support. Contact us to talk about what you need.

The cost depends on the services you need, how often you need them and how your care is funded. If you use Support at Home, the Australian Government funds your services within your budget. Depending on your income and assets, you may contribute to the cost of some services. You don’t contribute to clinical supports such as nursing care, and from 1 October 2026 you don’t pay a contribution for approved personal care services. If you’re a private client, we’ll explain the costs before your services begin. Contact us to talk through your situation.

Getting started

To apply for Support at Home, you can start the process by getting in touch with your GP, who can refer you to My Aged Care for an assessment, or you can contact My Aged Care on 1800 200 422 directly to create a profile and arrange an assessment.

If you have already gone through the process and would like to get started with our services, you can contact our team today to arrange an initial consultation over the phone or in person. We always prefer to meet and greet our clients before providing services.

At Anjelic Hearts, we are dedicated to providing exceptional services in the Sutherland Shire, St George region, Wollongong, Inner West Sydney, Eastern Suburbs and South Western Sydney.

Only if you’d like to use Support at Home funding, which requires an aged care assessment through My Aged Care. Private clients don’t need an assessment to receive our services. Either way, we like to meet you before providing services, so we can understand your needs.

Working with us

We take the time to understand your needs, preferences, routines and culture before care begins, and we consider these when arranging your support. If something isn’t working for you, tell us and we’ll look at what we can change.

Please let us know as early as possible if you need to cancel or reschedule a visit. Our cancellation terms are explained in your service agreement before care begins. If you have any questions, please contact us.

Yes. Our services are tailored to your needs, and you can contact us about your care plan at any time. We’ll review and update it with you as your needs change.
