Keeping track of medicines can be tricky, especially when they change. We provide practical support to help you take your medicines as your GP or pharmacist intended, and we let you and your family know if we notice anything that needs attention.

Talk with us

Contact us for an initial consultation over the phone or in person. We always prefer to meet and greet you before providing services.

We take the time to understand your needs, goals and preferences, and create a person-centred care plan with you and your family.

Care begins

Our team provides your agreed supports at home. You can talk to us about your care plan at any time, and we’ll adjust it as your needs change.

How this service is funded

Available through Support at Home, with our partner Trilogy Care, or privately. Your assessment and budget determine what Support at Home can fund. Whether you pay a contribution depends on the type of service and your income and assets.

Questions about medication management

What kind of medication support do you offer?

Support is tailored to your needs. It can range from reminders and help opening packs, through to nursing support where clinical input is needed. We’ll agree the right level of support with you when we build your care plan.

Let us know as soon as your GP or pharmacist changes your medicines, and we’ll update your care plan. Questions about the medicines themselves should always go to your GP or pharmacist.

Under Support at Home, help with taking medication can be provided as part of personal care, and nursing care is a separate clinical service. What you can receive depends on your assessment and approved services. Private clients are also welcome. Learn more about Support at Home.
