Anjelic Hearts Pty Ltd (ABN 18 697 958 357) respects your privacy. This policy explains how we collect, use, store and share personal information, including health information, and how you can access or correct it or raise a concern. We handle personal information in line with the Privacy Act 1988 (Cth) and the Australian Privacy Principles.

The information we collect

Depending on how you deal with us, we may collect:

  • your name, phone number, email address, postcode and other contact details
  • details you share in an enquiry, such as the type of support you’re looking for
  • health and care information needed to plan and provide your care, such as medical conditions, medicines, mobility needs and care preferences
  • information about your cultural background, language or religion, if you choose to share it so we can tailor your care
  • contact details for your family members, representatives, GP or other health professionals
  • funding information, such as whether you use Support at Home or pay privately
  • information about how you use our website, such as pages visited, collected through cookies and analytics tools.

Health information and information about cultural background or religion is sensitive information. We only collect it with your consent, or where the law allows.

How we collect information

We collect information directly from you when you contact us, complete our website form, email or call us, or meet with our team. With your consent, we may also collect information from your family or representative, your GP or other health professionals, or Trilogy Care if you receive services through Support at Home.

You can choose not to identify yourself when you make a general enquiry by email. However, we can’t plan or provide care without the information we need to do so safely.

Why we collect and use information

We use your information to:

  • respond to your enquiry
  • assess your needs and create your care plan
  • provide, coordinate and review your care
  • communicate with you, your family or representative, and professionals involved in your care
  • manage service agreements, invoicing and funding
  • meet our legal, safety and quality obligations
  • improve our services and website.

We won’t use your information to send you marketing unless you have agreed, and you can opt out at any time.

Who we share information with

We only share information where it’s needed and allowed by law. This may include:

  • Trilogy Care, if you receive services through your Support at Home package
  • your GP, pharmacist or other health professionals involved in your care, with your consent
  • your family members or representative, with your consent
  • service providers who help us run our business, such as IT, email and website hosting providers
  • government agencies or others where the law requires or allows it, or where it’s needed to prevent a serious threat to someone’s life, health or safety.

We do not sell your personal information.

Storing and protecting information

We take reasonable steps to protect your information from misuse, loss and unauthorised access, change or disclosure. Access is limited to people who need it to do their work. When we no longer need information and aren’t required to keep it by law, we take reasonable steps to destroy it or remove identifying details.

Some of our service providers, such as email or cloud storage providers, may store information on servers outside Australia. Where this happens, we take reasonable steps to make sure your information is protected.

Our website

Our website uses cookies and analytics tools to understand how visitors use the site and to improve it. This information doesn’t usually identify you. You can set your browser to refuse cookies, although some parts of the site may not work as intended. Our enquiry form uses spam protection to keep the form secure.

Accessing and correcting your information

You can ask to see the personal information we hold about you, or ask us to correct it if it’s inaccurate, out of date or incomplete. Please email info@anjelichearts.com.au. We’ll respond within a reasonable time. If we can’t give you access, we’ll explain why in writing, unless it would be unreasonable to do so.
