Talk with us

Contact us for an initial consultation over the phone or in person. We always prefer to meet and greet you before providing services.

Plan together

We take the time to understand your needs, goals and preferences, and create a person-centred care plan with you and your family.

Care begins

Our team provides your agreed supports at home. You can talk to us about your care plan at any time, and we’ll adjust it as your needs change.

How this service is funded

Available through Support at Home, with our partner Trilogy Care, or privately. Your assessment and budget determine what Support at Home can fund. Whether you pay a contribution depends on the type of service and your income and assets.

Questions about dementia care

How do you keep care familiar for someone living with dementia?

We take time to learn about the person: their routines, history, culture and preferences. We build this into the care plan and aim to keep the same team where possible, so support feels familiar and predictable.

Yes. Our in-home support, such as companionship and help with daily tasks, can give family carers time to rest, attend appointments or take care of other responsibilities. Talk to us about what would help your family.

Yes, through the services in your support plan. Support at Home doesn’t list dementia care as a separate service. Instead, people living with dementia can receive services such as personal care, social support, respite and nursing, depending on their assessed needs and approved services. We can talk you through how our support fits with your Support at Home budget, or arrange private services. Learn more about Support at Home.
