Support at Home is the Australian Government program that helps older Australians live safely and independently at home. Through our partnership with Trilogy Care, an approved Support at Home provider, you can self-manage your package, choose Anjelic Hearts as your service provider and have more say in how your budget is spent.

Get assessed by My Aged Care
If you’d like to use Support at Home funding, ask your GP for a referral to My Aged Care, or contact My Aged Care directly to create a profile and arrange an assessment.

Contact Anjelic Hearts
Already approved, or paying privately? Get in touch to arrange an initial consultation over the phone or in person. We always prefer to meet and greet you before care begins.

Start your care plan
We work with you, your family and, for Support at Home clients, Trilogy Care to build your person-centred care plan and begin your services.

Is Anjelic Hearts partnering with Trilogy Care?
Yes. Anjelic Hearts has partnered with Trilogy Care, an approved Support at Home provider, to ensure that you have access to high-quality services via your Support at Home funding. Trilogy Care can administer your care package while you have the flexibility to work with the service providers of your choice.
What is Support at Home?
Support at Home is a funding program provided by the Australian Government that supports older Australians to live safely and independently in their own home. It is designed to support you with daily activities you may need help with, such as cleaning, showering or attending appointments.
Do you take private clients?
Yes. As well as supporting people with Support at Home funding, we welcome private clients who pay for their own care. Contact us to talk about the support you need.
How do I get started?
To apply for Support at Home, you can start by talking to your GP, who can refer you to My Aged Care for an assessment, or you can contact My Aged Care on 1800 200 422 to create a profile and arrange an assessment.
If you have already been through this process, or you would like to use our services privately, contact our team to arrange an initial consultation over the phone or in person. We always prefer to meet and greet our clients before providing services.
